Assumption of Risk, Waiver and Release of Liability Agreement

By checking the box below and submitting this form, I acknowledge that I have read, understand, and voluntarily agree to the terms of this Assumption of Risk, Waiver and Release of Liability Agreement (“Agreement”) with Cedar and Sol LLC (“Company”).

I understand that Cedar and Sol LLC provides mobile sauna services with inherent risks related, including but not limited to: Wood-fired or electric sauna use, Cold plunge immersion with filtration system. I understand these services involve exposure to extreme heat, cold water immersion, and changing environmental conditions.

I acknowledge that participation in sauna and cold plunge activities involves inherent and potential risks, including but not limited to:

• Dizziness, fainting, dehydration

• Heat exhaustion, heat stroke

• Hypothermia or cold shock

• Irregular heartbeat or cardiovascular stress

• Slipping or falling on wet surfaces

• Burns from hot surfaces or steam

• Respiratory distress

• Aggravation of pre-existing medical conditions

• Serious injury, permanent disability, or death

I understand that these risks may arise from my own actions, the actions of others, the condition of the equipment, or the nature of the activities themselves. I voluntarily assume all risks, known and unknown, associated with my participation.

I certify that:

• I am at least 18 years of age

• I am physically and medically able to participate.

• I understand that sauna and cold plunge use may not be safe for individuals who are pregnant, have heart conditions, high or low blood pressure, cardiovascular disease, respiratory conditions, seizure disorders, diabetes, or other medical concerns.

• I understand that alcohol, drugs, or certain medications may increase risk.

I agree that it is my responsibility to consult a physician prior to participation if I have any medical concerns.

I agree to:

• Follow all posted guidelines and staff instructions

• Adhere to session time recommendations

• Enter and exit equipment safely

• Use provided seating pads and hygiene protocols

• Immediately exit if I feel unwell

I understand that failure to follow rules may increase risk and may result in removal from participation without refund.

To the fullest extent permitted under the laws of the Commonwealth of Massachusetts, I hereby release, waive, discharge, and hold harmless Cedar and Sol LLC, its owners, members, managers, employees, contractors, volunteers, agents, affiliates, and event hosts from any and all claims, demands, causes of action, damages, losses, liabilities, costs, or expenses arising out of or related to my participation, including claims based on negligence.

This release applies to personal injury, illness, property damage, disability, or death arising from my participation in the activities described above.

I agree to indemnify and hold harmless Cedar and Sol LLC from any claims brought by third parties arising out of my actions during participation.

I authorize Cedar and Sol LLC to contact my emergency contact provided and seek emergency medical treatment on my behalf if deemed necessary. I understand that I am responsible for any medical costs incurred.

This Agreement shall be governed by and interpreted under the laws of the Commonwealth of Massachusetts. Any legal action arising from this Agreement shall be brought exclusively in a court of competent jurisdiction in Massachusetts.

If any portion of this Agreement is found to be unenforceable, the remaining provisions shall remain in full force and effect.

By checking the box indicating my agreement and submitting this form, I acknowledge that:

• I have carefully read this Agreement in its entirety.

• I understand its contents.

• I understand that I am giving up certain legal rights.

• I voluntarily agree to be bound by its terms.

I understand that checking the agreement box constitutes my electronic signature and has the same legal effect as a handwritten signature.
